Appointments

For new and existing patients, we can usually set an appointment within 1-2 days of your request. Of course, if it is more urgent or an emergency, we can usually find a slot to get you in that same day.

In addition to our regular office hours, we also have a doctor on call 24/7. We can happily help you over the phone or meet you at the office (if needed) after regular office hours. We are also on call 24/7 at all of the local area hospitals, MidHudson Regional Hospital and Vassar Brothers Medical Center. Rest assured that if you have the misfortune of having a foot injury that requires and emergency room visit one of the Sims & Associates Podiatry doctors will be there to assist and care for you.

Online New Patient Intake Form

As a new patient to Sims & Associates Podiatry, there are several forms that we need filled out with your information and signed. For your convenience, we now have a secure, online intake form that allows you to quickly and easily fill out the forms and submit them online. This saves you from having to take this extra time filling out paperwork at our office before your appointment. In order to access our online intake forms, you will need an access code that we will supply to you once we call and confirm your first appointment. Once you have an authorization code you can login to the forms page and submit your information prior to your appointment.

If you’re not comfortable filling out forms online or prefer to fill them out and bring them with you, rest assured you can still do so. Download our ‘New Patient Packet’, print the forms, fill them out and sign them in the comfort of your home at your leisure and bring them with you to your first appointment.

Insurance

We participate and accept most commercial insurance plans. Please call our office to verify if we participate in your insurance plan. In the rare case that we do not participate in your insurance plan or if you do not have insurance, we are happy to work with you to develop other arrangements for payment of our services.
