Thank you for submitting your online appointment request to Sims & Associates Podiatry. Your request has been received by our office and one of our dedicated team members will be in contact with you shortly to confirm your appointment details and answer any questions you may have. When we call, our staff will confirm your information, set up your appointment and if you’re a new patient, they will provide access to our secure, online patient information form as well as set you up with access to our secure, patient portal.

IMPORTANT

If You’re a New Patient at Sims & Associates Podiatry (or have not been seen in the past three years) - there are several forms that we need filled out with your information and signed. For your convenience, we now have a secure, online intake form that allows you to quickly and easily fill out the forms and submit them online. This saves you from having to take this extra time filling out paperwork at our office before your appointment. In order to access our online intake forms, you will need an access code that we will supply to you once we call and confirm your first appointment. Once you have an authorization code you can login to the forms page and submit your information prior to your appointment.

If you’re not comfortable filling out forms online or prefer to fill them out and bring them with you, rest assured you can still do so. Download our ‘New Patient Packet’, print the forms, fill them out and sign them in the comfort of your home at your leisure and bring them with you to your first appointment.
